Mountain Park Health Center proposes to improve understanding of the health care system and create linkages to available community resources for Somali refugees through a new program entitled "Dhexdhexadiye". This program will utilize a community Dhexdhexadiye (which means liasion in Somali) to provide support to more than 600 Somali patients at the Mountain Park Health Center East Phoenix.
